ATTENTION 5TH GRADE PARENTS!!!
There was a packet of information sent home with your children regarding the ads, payments, and uploading pictures for the 5th Grade Memory Book. The deadline is FEBUARY 3RD!!!
The information can also be found on the Oakhurst PTA website at www.oakhurstpta.com under the 5th Grade Celebration tab.
Feel free to upload as many pictures as you would like as they will be used for the DVD as well.
The 5th Grade Celebration and Breakfast is funded through the Yearbook Ad Submissions!!! Please get your ads and payments in by the deadline posted!
There is also a page for your child to fill out…Class of 2012 Favorite Things. Please have your child fill this out and make sure they put their name on the page as well.
Everything should be turned into your child’s classroom by February 3rd!!!

Contribution Form
In order to make 5th Grade Celebration a success and to provide every student with a memory book, T-Shirt and DVD, we would like each family to contribute $30.00 per student. We would like to have the T-Shirts early in order to give them the opportunity to enjoy them for more than just the day of the celebration. Please send in your contribution in an envelope with your child’s name and teacher on it to their classroom by October 28th.
